Prevention of AKI and hypersensitivity reactions in adult patients undergoing iodinated CECT

The RCR National Re-audit of Prevention of Acute Kidney Injury (AKI) and Hypersensitivity Reactions in Adult Patients Undergoing Iodinated Contrast Enhanced CT (CECT) is based on recommendations from RANZCR 2018 guidelines (endorsed by RCR in 2019).

Background

The first National RCR Audit was conducted in 2015 (published 2017), and was based on ICWP and NICE guidance from 2013. There have been various changes to the pre-CECT screening of renal function and renal protection advice, following endorsement of the RANZCR guidance by RCR in 2019.

These changes resulted in a more selective – but much higher risk – group of patients requiring pre-CECT renal function check, and subsequently an even smaller, very high risk group needing renal protection.  Special consideration is also given on risk assessment, follow-up, and secondary prevention of hypersensitivity iodinated contrast media reactions and advice in special medical conditions to consider pre-contrast media administration.

Given the recent updates, and the fact that results of the 2015 audit revealed room for improvement, it is considered to be a good time to re-run the audit.

Aims

  1. All departments follow up to date RCR endorsed RANZCR 2018 guidelines on the prevention of Contrast Induced Acute Kidney Injury
  2. All departments follow up to date RCR endorsed RANZCR 2018 guidelines on risk assessment, follow-up, and secondary prevention of hypersensitivity iodinated contrast media reactions
  3. All departments follow up to date RCR endorsed RANZCR 2018 guidelines on advice in special medical conditions to consider pre-contrast media administration.
